Students & Parents/Guardians,

Welcome Back To School!  I hope you have enjoyed your summer and are ready for a wonderful school year! The first days of school can be, to say the least, caotic, but hopefully exciting.  During the first week of school, students will recived numerous forms and letters!!  To make things a little easier, I have provided the information you and your child will need regarding MEDICAL CARE at school.

1.  Health Profile/Consent Form (Return this form ASAP!)

  • Your child should have received this form with the "first day packet" of information.  This form provides myself and the school staff with vital information regarding your child so that we are able to take the best possible care of your child while they are at school.  Return this form to the homeroom teacher ASAP.

2.  Medical Conditions

  • If your child has a medical condition such as diabetes, asthma, severe allergies, etc., please make arrangements to meet with me so that the appropriate paper work, medications, and care plan can be reviewed and put into effect.

3.  Medications at School

  • BCMS does not supply medication for students.  Prescription medication must be brought in by the parent/guardian.  It must also have a label and be current in the last 30 days.  Appropriate consent forms must be signed by the parent/guardian.  Over-the-counter medications are permitted for the child to use at school when brought in by the parent/guardian and also needs the appropriate consent forms signed by the parent/guardian.
